Supporting older adults with respect, dignity and care

Dr Supriya Rama Krishnan (MBBS FRACP MForensMed)

Dr Supriya is a Geriatrician who works locally in Werribee Mercy Hospital across inpatients, outpatients, and community services. It involves all aspects of geriatric medicine. Dr Supriya regularly teaches and supervises medical students, junior doctors, and geriatric advance trainees. She consults in the inpatient medical and rehab ward at the St Vincent’s Hospital in Werribee. 

Dr Supriya is also the medical lead at the Grampians Health Continence Service at the Queen Elizabeth Centre, Ballarat. 

Dr Supriya enjoys providing comprehensive geriatric assessments and getting to know her patients, to maximize her patient-centered approach towards improving their health and well-being. Her friendly approach and emphasis on clear communication has often helped older adults feel at ease. Dr Supriya also communicates well in the Malay language (Bahasa Malaysia). 

After completing her geriatric specialist training, Dr Supriya undertook a year of clinical forensic medicine work in the Victorian Institute of Forensic Medicine and proceeded to complete the Master of Forensic Medicine from Monash University. 

Dr Supriya has an interest in complex case management and the medico-legal interface of ageing, including elder abuse and geriatric forensic. 

When she is not working, Dr Supriya enjoys spending time with her mechanic husband in his automotive workshop in Hoppers Crossing and their two dogs.

Dr Yih Harng Chong (MBChB FRACP PhD GChPOM)

Dr Yih Harng Chong is an experienced Geriatrician physician with current clinical appointments with Grampians Health. He has completed his training in New Zealand and worked in various subspecialty fields within Geriatric Medicine in both New Zealand and Australia. This includes perioperative medicine, and he is a Graduate of the Chapter of Perioperative Medicine with ANZCA. Dr Chong is an experienced Orthogeriatrician, acute geriatric medicine, and is involved with the cognitive and dementia clinic. 

Dr Chong has strong academic interest, and attained his PhD with the University of Otago, NZ in the health of the older male. Dr Chong is a senior lecturer with University of Melbourne, and previously held Senior Lecturer roles with University of Auckland and University of Otago medical schools. Dr Chong is actively involved in teaching spanning from medical students through to medical doctors in physician training. 

Dr Chong brings with him a compassionate holistic approach to the health of the older adult. Dr Chong has special interest in frailty, cognitive impairment, falls and osteoporosis management, polypharmacy, complex geriatric assessment, and others. Dr Chong is available for clinic consultation, home visits and residential aged care visits, and telehealth consultation. Dr Chong is well versed in English and multiple Chinese languages.

Dr Sara Sharma (MBBS FRACP)

Dr Sharma completed her Bachelor of Medicine and Bachelor of Surgery at James Cook University in Townsville, during which she grew her interests in Geriatrics in both City and Regional settings. Dr Sharma completed her Basic Physician Training in Brisbane at the Prince Charles Hospital and had the opportunity to be exposed to General Cardiology and Respiratory units as well as Adult Congenital Diseases and Heart and Lung Transplants in younger and well as older adults. This motivated her to complete a Certificate for clinical ultrasound, focusing on interpreting Heart and Lung ultrasound imaging. 

Following her transition into Advanced Training she gained skills in managing Acute Stroke as well as Post Stroke Rehabilitation and Syndromes in Toowoomba Hospital in Queensland. Following this, along with exposure to Age care Outreach and Memory Clinics, she also had the opportunity to work with Amputees in rehabilitation settings as well as Old Age Psychiatry.

Dr Sharma is passionate about healthy aging, including incorporating resistance training and protein fortification in older adults to prevent or reduce sarcopenia. Her other interests also lie in memory and mood concerns along with managing movement disorders, such as Parkinson’s disease, from a wholistic and person-cantered approach. She is also committed to professional development and is a member of Royal Australasian College of Physicians, Australian and New Zealand Society for Geriatric Medicine, Australasian Cognitive Neurology Association and Incontinence Society of Australia as well as attending regular conferences and meetings.

Dr Sharma is fluent in Hindi and speaks Punjabi as well.

Dr Sharma is available to see patients in Aged Care Facilities as well as Home Visits if needed.

Outside medicine, Dr Sharma’s interest include spending time with her family, cooking with diverse and protein rich whole foods, being physically active and painting.

Ms Heather Lambert (Medical Administrator)

In her career Heather has worked in fashion, hospitality, and as a pastry chef in well known Melbourne venue’s before transitioning into the healthcare sector where she has worked for over 20 years. 

Heathers various roles within the health service industry has focused on offering compassionate and respectful service to patients and their carers from diverse backgrounds. Heather takes pride in fostering meaningful connections, understanding individual needs, and providing unwavering support in a caring and professional manner. 

In her spare time Heather cherishes every moment she spends with family, friends and especially her two wonderful granddaughters. Heather is a lover of travel, live performances and enjoys expressing her creativity through baking and other artistic endeavors.
